Under the direction and supervision of a Registered Nurse or Licensed Practitioner, the LPN follows the nursing process: assessment, planning, implementation, evaluation. The LPN contributes to the assessment, planning, implementation, and evaluation of nursing care for a designated group of healthcare consumers, addressing spiritual, physiological, social, and psychological aspects of care.

• Conduct systematic, dynamic assessments, collecting and analyzing data such as psychological, physiological, sociocultural, spiritual, economic, and lifestyle factors.
• Assist in creating measurable short- and long-term goals in collaboration with the RN, patient, and multidisciplinary team.
• Implement and modify care plans using assessment data, clinical reasoning, standards of care, and evidence-based practice, including care coordination, education, and discharge planning.
• Document all phases of the nursing process in the patient’s record.
• Participate in ongoing professional development and maintain a safe, collaborative work environment.

• Graduate of an LPN nursing program.

• Active LPN licensure in Ohio or via the nurse licensure compact.
• Basic Life Support required upon hire.
• Accepted credentialing bodies include the American Heart Association.

• Knowledge of current nursing practice.
• Critical thinking, professional verbal and written communication, technical/computer skills.
• Attention to detail, customer service, adaptability, and confidentiality.
