What you will do:

  • Coordinate and manage Education, Health, and Care (EHC) plans, ensuring compliance with statutory requirements.
  • Serve as a point of contact for educational establishments, families, and health professionals to support seamless service delivery.
  • Conduct thorough assessments to devise tailored EHC plans that meet the needs of children and young people.
  • Collaborate with multidisciplinary teams to ensure cohesive and integrated care and education support.
