Job overviewThe Clinical Outreach Nurse is a key member of the Sexual Health Hertfordshire Outreach Team, responsible for planning, coordinating, and delivering innovative sexual health outreach services across Hertfordshire. Working under the supervision of the Outreach Team Lead, you will provide accessible, person-centred sexual and reproductive healthcare, reaching individuals and groups who may face barriers to accessing traditional clinic-based services.
You will undertake holistic SH assessments, deliver testing and treatment interventions, contraception advice, vaccinations, HIV prevention strategies and promote healthier sexual behaviours. Working collaboratively with community organisations, voluntary sector partners, education providers and healthcare professionals to identify needs, develop targeted outreach initiatives, and improve outcomes for vulnerable and underserved populations.
As a clinical expert, you will advocate for sexual health promotion, support prevention strategies, safeguard vulnerable individuals, reduce health inequalities, and strengthen engagement with communities. Contributing to service improvement, audit and the ongoing evaluation of outreach programmes to ensure high-quality, evidence-based care is delivered in line with national and local priorities.
As this is a community-based role, there is a requirement to hold a full, UK driving licence with access to a car for business purposes (unless you have a disability as defined by the Equality Act 2010)
Main duties of the job- Plan, coordinate and deliver sexual health outreach services across Hertfordshire, ensuring services are accessible, person-centred, and responsive to the needs of local communities.
- Provide clinical sexual and reproductive healthcare in community settings, including SH assessments, STI testing and treatment, contraception advice, vaccinations, and HIV prevention interventions such as PrEP, PEP and DoxyPEP.
- Deliver targeted outreach programmes to underserved and high-risk populations, supporting health promotion, risk reduction, and improved sexual health outcomes.
- Build and maintain partnerships with community organisations, voluntary groups, faith organisations, educational settings, and healthcare providers to increase engagement and access to sexual health services.
- Assess safeguarding risks and vulnerabilities, ensuring appropriate referrals and support are provided in line with Trust safeguarding policies and procedures.
