We are seeking an experienced Nurse Practitioner to join our growing palliative care program. This is an opportunity to make a meaningful impact by supporting patients with complex, chronic, or serious illnesses in a collaborative, patient-centered environment.

• Part‑Time: 20 hours per week (non‑benefited)
• Full‑Time: 32–40 hours per week (benefited)

• Minimum 1 year of experience managing complex patients, including independent prescribing experience
• Experience completing Face‑to‑Face (F2F) encounters for hospice patients
• Active Nurse Practitioner license in [State]
• Strong clinical assessment, documentation, and care planning skills
• Experience with eClinical
  
  Works (eCW) EMR preferred but not required
• Excellent communication and teamwork abilities

• Conduct comprehensive palliative care visits, including assessment, diagnosis, and management of symptoms
• Complete F2F encounters for hospice patients as needed
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Prescribe medications and coordinate care for patients with advanced or chronic illnesses
• Accurately document all encounters in the EMR
• Communicate effectively with patients, families, and other healthcare professionals
