Independent Living Case Consultant
Midstate Independent Living Choices invites applications for the Independent Living Case Consultant role.
Responsibilities- Demonstrate Independent Living (IL) philosophy in daily work.
- Provide core services:
Information & Referral – respond to inquiries, acquire and refer to community resources, and follow up to complete requests. - Advocate for consumer choice and consumer‑driven focuses in all situations; educate and support consumer interests, needs, and rights.
- Prepare for and participate in consumer‑related meetings; show awareness of and participate in systems advocacy.
- Keep current on legislation affecting persons with disabilities; be actively involved in committees and work groups impacting people with disabilities.
- Coordinate peer support and follow‑up services; conduct IL skills assessments and provide skills training in daily living areas.
- Assist with transition services to youth post‑secondary, nursing facility, and correctional environments.
- Support consumers in the development of IL plans; provide plan development with all consumers indicating a potential goal.
- Perform intake and maintain case files with mandated and required forms and documentation; provide coordination, follow‑along, and support for consumers with open files.
- Inform consumers of their right to accept or waive the plan.
- Staff MILC drop‑in centers as scheduled by the IL Department Manager, Peer Support Manager, and IL Program Director.
- Provide fee‑for‑service backup for other consultants; coordinate and maintain effective, positive working relationships with agency staff and other agencies/service providers to encourage referrals and promote cooperation.
- Attend monthly staff meetings, scheduled meetings with the IL Department Manager, and assist with coordination of adaptive community events.
- Join a staff workgroup/committee for quarterly in‑person meetings; provide program reporting as needed based on assigned projects.
- Attend conferences, trainings, work groups, both in‑ and out‑of the area of focus; prepare articles for the agency’s newsletter on a quarterly basis.
- Perform all other tasks as assigned.
- Experience working with people with disabilities; preferred or firsthand experience with a disability.
- Knowledge of Microsoft Office, Outlook email, and internet; working knowledge of Excel helpful.
- Knowledge of disability rights and the service system, benefits, entitlements, and supports utilized by people with disabilities.
- Knowledge of community resources available within the service area.
- Possess a valid Wisconsin driver’s license and proof of auto liability insurance meeting agency minimum limits.
- Ability to travel within MILC’s service area and statewide.
- Ability to maintain confidential information, including financial data and consumer information.
- Ability to pass a periodic WI Caregiver Background Check, including Department of Justice Criminal Record Check.
