Physical Therapy Assistant, Homecare

Job Description

Provide physical therapy services to clients in their home, under the direction of a physical therapist.

1.0 FTE;
Monday‑Friday 8:00‑16:30.

• Graduate of an accredited physical therapy assistant program
• Continuing education as required by state licensing boards
• Must have a Minnesota or Wisconsin Physical Therapy Assistant (PTA) license upon start date
• Must provide proof of MN and WI PTA license within 3 months of start if both licenses are not present on start date
• Must have a vehicle with auto insurance and a valid driver’s license and meet all criteria required under Lakeview’s Automobile Liability Guidelines
• Current BLS – American Heart Association or BLS – American Red Cross (CPR/AED for Professional Rescuer & Health Care Provider)
• Ability to read, write, and speak English

• Proof of application for alternate state within 4 weeks of start if both MN and WI PTA licenses are not present on start date
