Job Description
The Homecare RN provides direct and indirect nursing care. The nurse is accountable for the delivery of patient care utilizing the nursing process within the standards, policies, procedures and guidelines of the organization. Makes skilled nursing assessments of clients/caregivers in the homecare setting which includes physical, psychosocial, and environmental components. Develops, revises, and updates care plans in consultation with primary physician, client, and caregiver.
Coordinates all healthcare team members including client/caregiver to attempt to achieve the goals of the Care Plan.
32 hrs weekly / .8 FTE;
Mon-Fri 8am-4:30 pm (4 days/week), weekend rotation.
- Associate’s degree in nursing
- Must have MN or WI Registered Nurse (RN) license upon start date.
- Must provide proof of MN and WI Registered Nurse (RN) license within 4 months of start, if both MN and WI Registered Nurse (RN) licenses are not present on start date.
- Current BLS – American Heart Association or BLS – American Red Cross (CPR/AED for Professional Rescuer & Health Care Provider)
- Must have a dependable vehicle, valid driver’s license, auto insurance and meet all criteria required under Lakeview’s Automobile Liability Guidelines
- Bachelor’s degree in Nursing
- Proof of application for alternate state within 4 weeks of start, if both MN and WI Registered Nurse (RN) licenses are not present on start date.
