Physical Therapist

Full-time home-health PT for a locally owned Oklahoma agency. Company car and gas card. One-on-one visits. Pay per visit with expected full-caseload earnings of $85,000-$95,000.

Companion Healthcare is a locally owned, Medicare-certified home health, hospice, and private-duty agency with offices in Guthrie, Edmond, and Stillwater. We have been caring for Oklahoma families in their homes for decades. We are hiring a Physical Therapist to treat patients one-on-one after illness, injury, or surgery. This is field work, not a high-volume clinic. You set a field-based weekday schedule, drive a company car, and work with nursing, therapy, and case management behind you.

Pay and perks

• Pay per visit. Expected annual earnings of $85,000–$95,000+ on a full caseload.
• Company car and gas card. You are not putting work miles on your personal vehicle.

The work

• Evaluate and treat homebound patients across central Oklahoma:
  Oklahoma City, Edmond, Guthrie, Stillwater, Perry, and nearby communities.
• Build plans of care, teach patients and families home programs, and progress patients toward safe mobility at home.
• Coordinate with nursing, OT, ST, and case management. You are not working alone.
• Complete documentation within 24 hours of the visit. We provide a tablet and use Matrixcare.
• No regular weekends / no on-call or weekend/on-call expectation.

Who this is for

• Licensed Oklahoma PT, or eligible to obtain an Oklahoma license.
• Graduate of a CAPTE-accredited PT program.
• Current CPR and a valid driver’s license.
• Comfortable working independently in homes and then circling back with the team.
• Home-health experience is a plus, not required. Strong outpatient, hospital, or rehab PTs who want one-on-one care and a field schedule are encouraged to apply. New graduates with solid clinical internships will be considered; we will train you on home-health workflow.

We are not a national staffing mill. We are a local agency with nursing, therapy, and case-management support, plus private-duty and hospice under the same family of companies.
