You will play a key role in helping individuals sustain tenancies, develop life skills, and achieve positive outcomes as they transition into independent living.

Key Roles and Responsibilities:

• Deliver high-quality, trauma-informed, and person-centred support tailored to the needs of each young person.
• Provide flexible, intensive floating support within the community to promote independence and tenancy sustainment.
• Manage a caseload independently, including planning your own diary and prioritising support effectively.
• Support young people transitioning from supported accommodation into their own tenancies and local communities.
• Utilise case management systems (e.g. SHARP) to review, monitor, and evaluate progress against outcomes.
