Are you an experienced and compassionate Social Worker looking for your next opportunity within a hospital setting?

We are currently seeking a skilled Hospital Team Social Worker to join a busy multidisciplinary team in Stirling, supporting patients and their families to achieve safe, appropriate and person-centred outcomes.

As a Hospital Team Social Worker, you will work closely with patients, families, carers and a range of health and social care professionals to assess needs, coordinate support and facilitate safe and timely discharge from hospital.

• Undertaking comprehensive assessments of patients' social care needs.
• Developing person-centred care and support plans.
• Supporting patients through the hospital discharge process.
• Assessing risks and identifying appropriate care and support arrangements.
• Working closely with medical, nursing, occupational therapy and other multidisciplinary colleagues.
• Liaising with families, carers and external agencies to coordinate services.
• Arranging appropriate community-based support, care packages and placements where required.
• Contributing to safeguarding assessments and risk management.
• Advocating for patients' rights, choices and independence.
• Maintaining accurate and up-to-date case records.
• Participating in multidisciplinary meetings and discharge planning discussions.
• Ensuring practice is in line with relevant legislation, policies and professional standards.

We are looking for a qualified Social Worker with:

• A recognised Social Work qualification.
• Current registration with the Scottish Social Services Council (SSSC).
• Experience with in hospital, adult services, community social work or a similar setting.
• Strong assessment, care planning and risk management skills.
• Experience working within a multidisciplinary environment.
• Knowledge of adult social care legislation, safeguarding and hospital discharge processes.
• Excellent communication and interpersonal skills.
• The ability to manage a busy caseload and prioritise effectively.
• A professional, compassionate and person-centred approach.
• The ability to work confidently both independently and as part of a wider team.

Previous experience of hospital discharge or integrated health and social care services would be highly desirable.
