SSM Qualifications
  • Current Certificate of Competence from the College of Nurses of Ontario and current PLP.
  • Bachelor of Science in Nursing is an asset.
  • Strong working knowledge and proficiency in relevant computer applications (Microsoft Office, nursing care plan software such as Point Click Care).
  • Leadership experience and extensive knowledge of care requirements for seniors in long‑term care settings.
  • Customer service experience is an asset.
  • Demonstrates strong leadership, team building, interpersonal dynamics, and problem‑solving including conflict‑resolution skills.
  • Current CPR.
  • Fully vaccinated with an approved COVID‑19 vaccine and willing to adhere to IPAC recommendations.
  • Ability to communicate in Finnish and/or Estonian is an asset.
  • Must be able to motivate and direct the care team.
  • Must be able to relate to people of all ages and levels, both professional and public.
  • Must have sound critical thinking and resource management skills.
  • Must be able to commit to and meet repeated deadlines.
  • Excellent verbal and written communication skills.
Responsibilities
  • Provides clinical oversight to care staff (RPNs, PSWs) during shifts, ensuring residents’ needs are met and unit activities are carried out.
  • Facilitates resolution of care staff issues, including intervening and reporting interdepartmental conflicts that may occur during the shift.
  • Ensures appropriate action for work‑related injuries and other employee incidents that occur during the shift, informing management as soon as possible.
  • Notifies ADOC/EDOC of any performance or safety issues in writing.
  • Replaces care staff where necessary in the absence of the scheduling clerk.
  • Authorizes shift exchanges that are urgently needed.
  • Seeks direction from ADOC/EDOC for authorization of overtime.
  • Provides input to the EDOC for the acquisition or purchase of supplies and other equipment needed for resident care.
  • Ensures appropriate action for equipment failures during the shift.
  • Completes or ensures clerical activities are performed in the absence of clerical support.
  • Communicates relevant information to oncoming care staff through a taped report, where required.
  • Completes the section relating to her shift on the 24‑hour report and submits it to the ADOC/EDOC the following day.
  • Establishes regularly scheduled MKK unit meetings with care staff during the shift and chairs the meetings.
Nursing Responsibilities
  • Assesses, plans, implements and evaluates care provided by herself/himself and other staff to ensure standards of nursing practice, MOH LTC guidelines and OFRA policies are maintained.
  • Completes admission profiles and discharges for residents.
  • Ensures an interdisciplinary care conference is scheduled annually for each resident and chairs the meeting; schedules additional conferences as needed.
  • Assesses daily need for restraint of identified residents.
  • Ensures appropriate action for any resident‑related incident (e.g., injuries, medication errors) that occurs on shift.
  • Acts as liaison between residents, their physician and other providers; assists attending physicians during visits.
  • Manages infection control issues on shift and ensures appropriate action following the issue.
  • Acts as liaison between residents and other OFRA services (food, laundry, housekeeping); refers issues to ADOC/EDOC as necessary.
  • Facilitates resolution of resident/family conflicts; refers issues to ADOC/EDOC as necessary.
  • Ensures accurate and complete documentation by care staff.
  • Completes accurate resident records and other reports (scheduled chart reviews, death reports) per standards and policies.
  • Ensures relevant practitioners complete annual physicals, 3‑month medication reviews and quarterly summaries.
  • Reviews and cosigns new MAR sheets after RPN’s initial check.
  • Ensures registered staff complete narcotic counts at the beginning and end of each shift and witnesses drug destruction.
  • Provides PRN health teaching to care staff about residents’ health conditions and needs.
  • Participates in nursing practice meetings.
  • Maintains regular communication with ADOC/EDOC, consulting on staffing and risk issues.
  • Continues professional development per College of Nurses of Ontario standards through continuing education.
  • Other duties as assigned by ADOC/EDOC.
