Overview

The philosophy of a Family Care Team (FCT) is to deliver care that is patient‑centered, comprehensive, team‑based, culturally safe, coordinated and accessible while focusing on quality and safety. This philosophy supports providers and care teams to meet clients where they are using an inter‑professional collaborative approach where care is provided in the right place, at the right time, by the right provider in a manner that best suits the client’s need.

The team will ensure clients are treated with respect, dignity and compassion, and enable strong, trusting relationships with providers and staff. FCTs enable delivery of a range of quality care for clients within their own communities while facilitating interdisciplinary work among healthcare providers. The FCT for the Baie Verte / Springdale Family Care Team will cover both communities and surrounding areas.

This position will be based in Springdale.

There may also be a requirement to support services such as, but not limited to, Acute Care Services, Long‑Term Care and Emergency Services which may include the Fast Track Stream of Care and Virtual Care Stream, and community teams.

Definition of Work

As part of the collaborative team, the Nurse Practitioner will practice to their full scope of practice to provide comprehensive assessment and care to clients and provide clinic leadership. The FCT aims to provide continuity of care to improve health outcomes and quality of life through client attachment and client paneling to clinic providers, with the support and wrap‑around services of the full team.

The successful candidate works independently in diagnosing conditions, prescribing medications, performing or ordering diagnostic tests and treatments, evaluating treatments and providing follow‑up care in collaboration with the interdisciplinary team. The Nurse Practitioner provides comprehensive and holistic nursing care to clients including health promotion, injury and illness treatment and prevention, supportive, curative and rehabilitative care in a primary care practice setting.

The Nurse Practitioner will document using the Electronic Medical Record (EMR). In this leadership role, the Nurse Practitioner will contribute to the ongoing program development, implementation and evaluation of the FCT including client and clinician engagement. The role will encompass participating in clinic evaluations and continuous quality improvement processes based on current best practice guidelines. Flexible hours are required (combination of days, evenings and weekends).

The Nurse Practitioner will report to the Regional Director of Primary Health Care & Community Nursing in the Central Zone.

As an advanced practitioner, the Nurse Practitioner functions as practitioner, educator and leader. As governed by provincial regulations, he/she practices independently, collaborating with the primary care physicians in the area for issues outside the Nurse Practitioner scope of practice. Travel throughout the region may be required. The successful candidate must demonstrate an awareness of and be responsible for promoting and supporting person‑and‑family‑centered care, demonstrate a commitment to safety and partner with all team members to improve worker and patient safety by integrating safety into work practices, identifying risks and implementing improvements.

Required Qualifications
  • Graduation from a Nurse Practitioner program approved by Central Health.
  • Current registration as a Nurse Practitioner with the Association of Registered Nurses of Newfoundland and Labrador.
  • Current BLS certification.
  • Demonstrated knowledge of home and community health services.
  • Demonstrated exceptional leadership qualities and problem‑solving skills as well as oral and written communication skills.
  • Demonstrated ability to work well independently and in an interdisciplinary team environment.
  • Valid Newfoundland and Labrador Class 5 Driver’s License.
  • Consistent demonstration of the core values of NL Health Services:

  • Satisfactory record of work performance and attendance.
  • Satisfactory Certificate of Conduct from the RCMP or local police authority.
Preferred Qualifications
  • Demonstrated skill in using health‑care‑based computer systems (CRMS, Meditech, EMR).
  • Experience in the care and management of patients with diabetes, COPD, heart failure and/or mental health disorders.
  • Experience working in a Primary Health Care setting.

Electronic Medical Records (EMR) training is required and will be provided upon hire.
