Join the UHN Reactivation Care Centre as a part-time Registered Practical Nurse in the Transitional Care Program. Contribute to patient care and team collaboration while enhancing your nursing skills.

This role offers the chance to work with a multi-disciplinary team dedicated to transitional care. You will be responsible for planning, managing, and evaluating nursing care in accordance with industry standards while mentoring students and new staff. This position is part of a talent pool for various nursing opportunities, allowing for flexibility in employment types.

Key Responsibilities:

• Plan and evaluate nursing care for patients

• Implement patient and family care based on team input

• Participate in interdisciplinary care planning forums

• Mentor RPN students and new staff

• Engage in professional development and initiatives

Requirements:

• Valid registration with the College of Nurses of Ontario

• Diploma in Practical Nursing

• Recent related clinical experience preferred

• Effective problem-solving skills

• Demonstrated commitment to professional development
