AboutThe Role
The Clinical Nurse Navigator serves as the coordinator of care for adults with congenital heart defects, acting as a dedicated liaison between inpatient and outpatient settings. This role provides support, education, referrals, and coordination of care to patients and their families prior to, during, and following their healthcare experience. Working closely alongside nursing staff and APPs, the Navigator facilitates integrated care through communication with the interdisciplinary team, collaborates on staff education, and promotes evidence-based clinical practices that exemplify the pursuit of clinical excellence.
We are looking for a self-starter who is passionate about adult cardiology and committed to optimizing patient satisfaction and achieving the best possible outcomes.
- Serve as the primary liaison between inpatient and outpatient care teams for adults with congenital heart defects
- Coordinate efforts between physicians, healthcare administration, families, community resources, and staff while tracking patient progress throughout the continuum of care
- Support nursing staff and APPs in coordinating plans of care and managing care transitions
- Provide education, referrals, and support to patients, families, staff, and physicians surrounding the specialty
- Facilitate integrated care through communication with strategic members of the interdisciplinary team
- Establish and maintain strong lines of communication with patients, families, clinical teams, and leaders to optimize patient satisfaction
- Document concisely, accurately, and timely
- Participate in the development and implementation of evidence-based practices and educational materials
- Support local and system-wide processes and participate in team member development activities
- Active RN licensure
- Bachelors of Science in Nursing
- Self-motivated with strong communication and interpersonal skills
- Experience working with adults or pediatric patients with congenital heart defects
- Cardiac certification: CCRN or ANCC or ability to obtain certification within 6 months of hire
