Licensed Practical Nurse, Home Health

Center Well Home Health is seeking a licensed practical nurse to deliver personalized, one‑on‑one care to patients in the comfort of their homes.

$44,600 – $61,400 per year.

• Provide professional nursing services under RN supervision in compliance with the state’s Nurse Practice Act, licensure/certification requirements, health‑care standards, and Center Well policies.
• Deliver skilled nursing interventions for patient illness, rehabilitation, and preventive care using a holistic approach aligned with the plan of care.
• Apply clinical knowledge to facilitate problem resolution and achieve individualized patient goals.
• Consult with RN supervisor regarding changes to the treatment plan and communicate permitted verbal orders from physicians to the RN supervisor.
• Implement the nursing care plan using a systematic, goal‑driven approach.
• Maintain timely communication with patients, physicians, managers, and the health‑care team regarding patient needs, status changes, and discharge planning.
• Maintain professional conduct concerning confidential information, sharing data only when needed and in the patient’s best interest.
• Ensure proper use of supplies, materials, and equipment to safely perform the plan of care.
• Document and submit required records accurately, including shift notes for day services rendered.

• Graduate of an accredited Licensed Practical Nursing Program or accredited school of vocational nursing.
• Current LPN license in the practicing state.
• Valid driver’s license, auto insurance and reliable transportation.
• Current CPR certification.
• At least two years of LPN/LVN experience in a clinical setting, preferably home health or hospice.
