Gentiva Hospice seeks an experienced Registered Nurse Case Manager to deliver personalized hospice care in patients' homes or preferred settings. You will conduct initial and ongoing assessments, implement and update the Plan of Care, and collaborate with physicians, LPNs, CNAs, social workers, chaplains, and others to ensure compassionate, high-quality care.
You will educate families, coordinate with the care team, and document care plans and visits.
