The Trafford and Stockport College Group (TSCG) is seeking experienced SEND Transition Coordinators, contributing to our vision of unlocking potential for successful futures.

We are looking for two dedicated SEND Transition Coordinators and their roles will involve working with young people, their families and external partners to support their transition into the College and support them to succeed in meeting their potential.

• Manage a dataset of feeder school SENCO and Local Authority SEND Officers.
• Ensure procedures and practices are clear and support an effective person‑centred transition process for learners, which is fully compliant with the College’s duties under the SEND Code of Practice and the Children and Families Act.
• To establish strong partnership working with local schools, SENCOs in particular, and other external agencies involved in transition support for school leavers.
• Develop rapport and confidence with the young people, their families, carers and professionals, in line with the College’s duties under Children and Families Act and the SEND Code regarding transition.
• Promote the College’s commitment to the Department for Education Preparation for Adulthood agenda, by providing advice on the College’s curriculum and support offer, which allows learners to make informed decision about next steps.
• Work closely with Careers, Guidance and Admissions teams and curriculum areas to advise on learners’ support requirements and act as an advocate for the student and family if appropriate.
• Provide support for Open Evenings, taster days, initial student‑centred interviews, enrolment and induction, as appropriate to the learner’s needs and preferences.
• Attend transition reviews for Year 11 applicants and contribute appropriately to devising EHCP outcomes for post 16 settings, which are achievable and aspirational.
• Produce Transition Plans for each learner with an EHCP, which includes an initial student profile of aspirations, achievements, likely support needs and course choices.
• Collate and store all relevant documentation associated with needs assessments, including existing EHCPs, to build a profile for additional learning support services to use to devise a suitable support package for applicants.
• Work with the Learning Support Manager and Learning Facilitator Coordinators to draft initial support plans and applications for high needs funding if appropriate.
• Disseminate transition information to the Director of Progression and Inclusion, Learning Support Managers and Learning Facilitator Coordinators in a timely manner and keep them abreast of updates and changes in relation to learners transitioning to the College.

• You should have a strong understanding of the challenges faced by young people with SEND aged 16-24 and be knowledgeable about the services available to support them. Your experience in a Further Education setting and your ability to help students achieve their goals will be crucial in this role
• You should possess excellent written skills, including spelling, grammar, and the ability to write concisely and accurately. Your ability to work with a diverse group of learners and develop successful strategies to meet their needs is essential
• You will be a role model for our students, demonstrating the values of ambition, resilience, collaboration, inclusivity, and respect
• In addition, you should have the ability challenge learners appropriately, set targets, monitor progress, and develop strategies to support their retention, attendance, and achievement
• Your well‑developed communication skills and ability to empathise and provide support to young people will be key to your success in this role.
