About this job

Main Duties and Responsibilities The post holder will work as part of The childrens Community Learning Disability Team (CCLDT) to deliver therapy and care programmes set by the CCLDT multi-disciplinary team.

The post holder will use their own knowledge and expertise to consider the child holistically and incorporate appropriate activities into sessions to meet the childs wider developmental needs, including communication, physical skills, sensory needs, play skills and positive behaviour support plans.

The post holder will offer ongoing assessment of childs needs, liaising with relevant professionals regarding progress and adjust behaviour plans and delivery accordingly.

Communications

Ability to communicate effectively face to face and via phone, email, video consultations and written letters.

Liaison with other professionals e.g. health visitors, Portage service, dieticians, school staff and social workers.

Liaison with families regarding appointments and behaviour plans.

Responsibility for Patient Care

Ongoing assessment of childs needs during behaviour support plans, liaising pastors relevant professionals regarding progress and adjusting the plan accordingly alongside nursing team.

Working autonomously in the community and managing their own case load.

Carrying out observations of children in different settings such as home, school and respite units.

Providing parents with skills and knowledge to deliver behaviour strategies at home.

Providing school staff with skills and knowledge to delivery behaviour plans in education settings.

Providing written reports on progress and sharing with parents and professionals as appropriate.

Adapting delivery to meet individual needs (e.g. face to face, group, video calls) to achieve optimal outcomes.

Being adaptable and flexible with location of delivery of care the service may need to be delivered at in clinic, home, schools or other locations
- Facilitating Riding the Rapids alongside other trained professionals virtually, face to face and in schools.

Supporting families with application and referral forms to access other services.

Planning and organising

Planning own diary and liaising with the Specialist Nurses on the team to meet needs of service.

Participating in meetings such as TAC meetings.

Organising appointments for patients in timely manner.

Managing room availability and suitability.

Making and providing resources/behaviour plans for parents/carers and educational settings as appropriate.

Seeking additional information from parents and CCLDT team to contribute to planning and delivery of behaviour plans.

Supporting Specialist Nurses in training sessions.

Attending MDT meetings such as CCLDT monthly referral meetings.

Responsibilities for Physical and / or Financial Resources

Infection control procedures including cleaning toys and maintain room cleaning log.

Maintaining safety and quality of equipment used e.g. toys, electrical equipment.

Developing and making resources for sessions as required e.g.

Board maker symbols.

  • Ensuring appropriate equipment is available for sessions.

Responsibility for Policy and Service Development and Implementation

Development of resources for use within the CCLDT.

Evaluating effectiveness of Plans e.g. evaluation forms for parents.

Contributing to service development meetings.

Flexibility to adapt within the role to meet service demands.

Developing and providing leaflets and information packs for parents and carers.

Completing risk assessments as necessary.

Responsibilities for Human Resources and Leadership
- Being aware of and following HR policies.

Working as part of a supportive team, being mindful of individual needs and working flex ahau to support other team members.

Adhering to Trust values.

Responsibilities for Teaching and Training

Maintaining continuing professional development and attending training courses as required for the role.

Disseminating information gained through CPD with other team members.

Delivering teaching and training to others e.g. teaching parents and education staff new skills within sessions, supporting the delivery of parent and education staff training courses.

Providing placements for volunteers and students, ensuring they adhere to Trust policies and procedures.

Facilitating observations of sessions for other professionals.

Supporting new members or less experienced members of staff.

Responsibilities for data and information resources

Recording contacts and non-attendance.

Keeping accurate and concise case notes.

Keeping CCLDT data base up to date for monthly referral meetings.

Research, Development and Audit

Use Datix to report incidences as appropriate
- Participation and support in research projects, as appropriate Physical Skills and Effort
