Job Summary
The Stockport Primary Care Network is transforming the delivery of health and social care services across Stockport. This role is part of our integrated teams working across Adult Social Care and the Stockport Foundation Trust to deliver person‑centred care 7 days a week.
Main DutiesAs a Nurse Associate Practitioner you will support the District Nursing Service by providing high‑quality care in patients’ homes and residential settings. You will work closely with Community Nurses and Adult Social Care staff to deliver a first‑class service to the residents of Stockport.

- Deliver nursing care in patients’ homes and residential services, covering 8 am‑6 pm, 7 days a week.
- Provide coverage for busy treatment rooms when required.
- Communicate effectively with patients, families and multidisciplinary teams.
- Maintain up‑to‑date nursing knowledge and support the integration of services within the Stockport Primary Care Network.
- Work independently, driving with a personal vehicle and keeping the vehicle available for duty.
- Promote safeguarding and child protection, following Trust policies and procedures.
- Undergo an enhanced DBS check and join the DBS update service.
- Registered Nursing Associate on the NMC Register with a Foundation Degree OR an AP degree.
- GCSEs (or equivalent) Grade C or above in Maths and English Language.
- Community nursing experience.
- Valid car driving licence and access to a vehicle.
- Excellent communication, interpersonal and organisational skills.
- Experience in district nursing or similar community setting.
- Knowledge of the Stockport Primary Care Network and Integrated Community services.
