An exciting opportunity has arisen to join the Children's Community Nursing team here at Stockport for a 9‑month fixed term or secondment covering maternity leave.
Key Responsibilities- Deliver care to children with acute and chronic health needs within their own homes across 7 days.
- Assess, plan, and implement care and evaluate outcomes.
- Promote safeguarding of children by implementing Trust policies and acting promptly on concerns.
- Communicate effectively with children, families and multidisciplinary team, adapting style to meet developmental, cultural and emotional needs.
- Manage a daily caseload, balancing competing demands and changing priorities.
- Plan, coordinate and organise care delivery to ensure continuity.
- Organise own workload and contribute to team workload planning.
- Participate in service planning, development and quality improvement initiatives.
- Use Trust resources efficiently and ensure safe use, storage and maintenance of clinical equipment.
- Identify and report issues relating to equipment, stock or resource availability.
- Contribute to cost‑effective care delivery through appropriate use of resources.
- Implement Trust, local and national policies, procedures and clinical guidelines.
- Contribute to the review, development and implementation of policies, pathways and protocols.
- Support the implementation of change to improve patient outcomes and experience.
- Supervise, mentor and support junior nursing staff, support workers and students.
- Act as a role‑model, promoting professional standards, equality, diversity and inclusion.
- Contribute to recruitment, induction and retention of staff where appropriate.
- Support the education and development of students and other staff.
- Deliver training, teaching and health education to children, families and carers.
- Participate in the delivery of in‑service training and sharing of best practice.
- Maintain professional development and mandatory training requirements.
- Maintain accurate, contemporary and confidential patient records using electronic systems.
- Ensure compliance with information governance and data protection legislation.
- Collect and input data required for service monitoring, reporting and evaluation.
- Use information and data to inform clinical practice and service improvement.
- Participate in audit, service evaluation and quality improvement activities.
- Support implementation of research findings and evidence‑based practice.
- Use audit outcomes to inform changes in practice and improve care delivery.
- Maintain awareness of current research and professional developments relevant to children’s community nursing.
- Undertake clinical procedures requiring precision, dexterity and accuracy.
- Travel independently across a range of community settings, including homes, schools and clinics.
- Manage the physical demands of community nursing, including moving and handling equipment.
- Maintain concentration and emotional resilience when caring for children with complex or distressing needs.
- Experience working with children who have acute health needs.
- Understanding of children with complex health care needs.
- Ability to work flexibly between 8 am and 9 pm, 7 days a week, including support for an overnight on‑call service for children at end of life.
- Strong safeguarding and communication skills.
- Experience with Trust policies, procedures, and clinical guidelines.
- Leadership and supervision experience (preferred).
