Locum Practice Nurse: Chronic Disease & Primary Care

Locum Ready is seeking a qualified Practice Nurse for a GP practice in Yarm. The role focuses on conducting chronic disease reviews, wound care, immunisations and general treatment room duties in a structured primary care setting.

You'll manage patient recalls, document in EMIS or similar systems, and contribute to NHS Health Checks and lifestyle assessments. Relevant DBS is required, and immunisation/cytology training is desirable.
