• MD or DO degree from an accredited medical school.
• Board Certified in Family Medicine.
• Current Illinois medical license or eligibility to obtain one.

• Compensation Range: $274,213 - $384,875. Salary based on experience and qualifications.
• Productivity + Quality bonus with 2-year income guarantee, and malpractice insurance.
