Position Overview
The Social Worker provides psychosocial and discharge planning services to all inpatients and select outpatients identified through the Utilization Review Program. Responsibilities include planning, organizing, and implementing social work, discharge planning, and post-acute care services for patients and families through patient and family engagement based on their goals and preferences. The role demonstrates proficiency in navigating community services to reduce barriers, identifies patients at risk of abuse, neglect or exploitation and intervenes per California mandatory reporting, collaborates with Resource & Utilization Management staff to assure appropriate services in a cost-efficient manner, coordinates services to promote continuity post-discharge, and delivers care to assigned age‑specific patient populations while participating in related continuing education.
Responsibilities- Plan, organize, and implement social work, discharge planning, and post‑acute care services.

- Collaborate with Resource & Utilization Management staff to assure appropriate, cost‑efficient services.
- Coordinate services to promote continuity of care post‑discharge.
- Deliver care to assigned age‑specific patient populations (infants, adolescents, adults, geriatrics, pediatrics) and participate in continuing education.
- Graduate of an accredited college or university with a baccalaureate or advanced degree in social work; MSW preferred.
- 1–3 years of related experience preferred.
FTE: 1.0 (5 x 8s) Day shift, 8:00 AM – 4:30 PM. Full‑time. Salary $31.40 – $39.25 per hour.
Location:
Stockton, CA. This is a non‑union position.
