Location

Position:
Registered Nurse – Service in Stockton and surrounding areas.

Compensation
  • Hourly and Paid Per Visit (PPV) hybrid model (negotiable based on experience, skill level, availability)

Patient Care
  • Completes initial, ongoing comprehensive patient and family assessment to determine hospice needs; provides physical assessment and history.
  • Provides professional nursing care utilizing nursing process as defined by state Nurse Practice Act.
  • Assesses and evaluates patient status, writes and initiates plan of care, re‑evaluates regularly, and revises plan as needed.
  • Uses health assessment data to determine nursing diagnosis.
  • Develops care plan with goals based on nursing diagnosis and includes patient and family in planning.
  • Initiates preventive and rehabilitative nursing procedures; administers medications and treatments per physician's plan.
  • Counsels patient and family on meeting nursing and related needs.
  • Provides health care instructions to patient as appropriate.
  • Assists patient with activities of daily living and facilitates self‑sufficiency or optional comfort care.
  • Acts as case manager when assigned; coordinates patient care for caseload.
Communication
  • Completes, maintains, and submits accurate clinical notes; records pain/symptom management changes.
  • Communicates with physician regarding patient needs and reports changes; obtains orders as required.
  • Communicates with community health personnel to coordinate care plan.
  • Teaches patient and family self‑care techniques; provides medication, diet, and other instructions per physician guidance.
  • Works cooperatively with interdisciplinary group.
  • Provides safe environment for patient.
  • Assists patient, family, and team in continuity of care.
  • Cooperates with family and interdisciplinary group to meet patient and family emotional needs.
  • Attends interdisciplinary group meetings.
Additional Duties
  • Participates in on‑call duties per policy.
  • Ensures necessary equipment and services are available.
  • Supervises ancillary personnel and delegates responsibilities.
  • Pursues personal growth and development; maintains professional knowledge through continuing education.
  • Fulfills requested and/or accepted case assignments.
  • Actively participates in quality assessment and performance improvement teams.
Qualifications
  • Graduate of National League of Nursing accredited school of nursing.
  • Current RN license in California and current CPR certification.
  • Minimum of one year of professional nursing experience within last three years, or bachelor’s degree with RN license (preferred). Minimum two years’ experience with at least one year in public health, home care, or hospice nursing preferred.
  • Valid driver’s license and insured automobile in good working order.
  • Management experience not required; responsible for supervising hospice aides.
  • Self‑directed; able to work with minimal supervision.
  • Excellent observation, verbal and written communication, problem‑solving, basic math, and competency checklist nursing skills.
  • Physical capability for prolonged walking, standing, lifting, transferring patients, lifting supplies; adequate vision and hearing to perform nursing skills.
