By joining our team, you and your patients have access to our local team of experienced professionals, which includes MDs, NPs, and RNs.

This position can be part-time or full time hours.

• Conduct comprehensive Annual Wellness Visits in compliance with CMS guidelines.
• Perform health risk assessments and screenings.
• Review current medications, reconcile lists, and assess adherence.
• Identify risk factors, gaps in care, and potential health concerns.
• Provide patient education on preventive health measures and chronic condition management.
• Document all encounters accurately and timely in the EMR system.
• Utilize tools to manage clinical and financial resources.

• Current, unrestricted Nurse Practitioner license in California
• Training or interest in geriatrics welcome, but not required.
• Current DEA license.
• Board Eligible or Board Certified in Family or Internal Medicine.
• Excellent communication, documentation, and interpersonal skills.
