Primary Care Nurse Practitioner | Geriatrics

We’re working on behalf of a healthcare organization whose PACE (All-Inclusive Care for the Elderly) program provides seniors with the opportunity to continue living in their homes and in their communities. They employ a collaborative interdisciplinary team (IDT) approach to evaluate and guide participant care, which is key to organizational values, team culture, and mission.

We seek a California licensed Family Nurse Practitioner with two (2) or more years of experience with the geriatric population for the role of Nurse Practitioner/Primary Care Provider. The person in this role will perform in-person comprehensive assessments including initial, semi-annual and unscheduled assessments, and continuous medical coverage to participants. She/he will collaborate with physicians and IDT to develop care plans for each participant.

She/he will collaborate with physicians and IDT in discharge planning from acute or long-term settings. She/he will help maintain care of participants in the nursing home, making regular visits as dictated by participant need. She/he will work collaboratively with physicians, community-based PACE physicians, and IDT to oversee and ensure effective care coordination.

Candidates must be a graduate of an accredited school of nursing and be licensed as a Nurse Practitioner in the state of California. Must have at least two (2) years of experience working with the senior population. Must possess current knowledge of chronic care/geriatric issues and best practices. Must have valid State Driver’s License with a clean DMV record. Copy of recent Vehicle Insurance required.

Due to the role’s work with the geriatric population, proof of vaccination prior to employment is required.

The role offers highly competitive compensation (salary $182,000 - $254,000 based on experience) and Medical insurance coverage (Medical, Dental, Vision) as well as 17 days of personal time off (PTO), 12 holidays observed annually, and sick time. 401 K savings + match also offered.
