Home Health RN – Welbe Health

Welbe Health is looking for a dedicated Home Health RN to provide skilled nursing care in the homes of seniors as part of our PACE program.

• Conduct Home Health Nursing Assessments to determine equipment needs, including participant preferences and goals, and actively participate in IDT meetings to develop care plans.
• Deliver and document home‑health nursing interventions as agreed upon, including maintaining a safe environment, promptly responding to physician orders, and correctly administering medications and performing ordered tests.
• Instruct participants, families, and caregivers regarding disease processes, self‑care techniques, and prevention strategies related to their conditions.
• Track and monitor Home Health scheduling in collaboration with the Home Health Manager or designated point of contact.
• Collaborate with the Welbe Health physician and other IDT members to assist in smooth care transitions between settings.

• Graduate of an accredited School of Nursing.
• Unencumbered RN license required.
• Minimum of one (1) year of nursing experience in a clinical setting with a frail or elderly population; home‑health agency experience preferred.
• Reliable means of transportation.

Salary/Wage base range: $47.74 – $63.02 hourly, plus bonus and equity. Actual pay will be adjusted based on experience and other qualifications.
