Responsibilities- As a Pro Health RN CM, you will:
Complete the initial assessment of a client and family to determine care needs - Regularly re-evaluate the client’s nursing needs
- Initiate the plan of care
- Identify discharge planning needs of the patient
- Coordinate patient care for assigned caseloads
- Minimum of two years professional RN experience
- Minimum of one year working in a Hospice setting
- Minimum of one year working in case management
- Current RN license to practice in CA
- Current CPR certification
- Current CA driver’s license plus proof of car insurance
- Computer/Internet experience (web-based charting to be used)
