Our value‑based PACE program allows our Primary Care Providers to provide truly comprehensive, holistic care so you can focus on what really matters: your patients.
Benefits- Competitive compensation of $181,749 – $254,499 with quarterly bonus
- Generous time off: weekends off, 17 days PTO, 6 sick days, and 12 paid holidays

- Regular hours: 8 a.m.–5 p.m., Monday through Friday, including home visits
- Rotating on‑call schedule—few nights or weekends per month, available by phone only

- Licensed Nurse Practitioner or Physician Assistant
- At least two years of experience working with the elderly
- Prescriptive authority and DEA registration
- Eligible to serve Medicare/Medi‑Cal participants
