Main duties of the job Undertaking routine and urgent clinical consultations, both face-to-face and remotely.

Assessing, diagnosing, treating, and managing a wide range of acute and long-term health conditions.

Managing a personal patient list, promoting continuity of care and building strong therapeutic relationships.

Providing health promotion, preventative care, and patient education.

Prescribing medications and reviewing treatment plans in line with current clinical guidelines.

Reviewing laboratory results, clinical correspondence, and patient records using EMIS Web and Anima.

Working closely with the practice's multidisciplinary team, including Clinical Pharmacists, Care Coordinators, Physician Associate, and GP Assistant.

Contributing to quality improvement initiatives, clinical audits, and service development projects.

Supporting the training and development of colleagues where appropriate.

Participating in practice meetings and contributing to the strategic development of the practice.

Maintaining accurate, timely, and comprehensive clinical records.

Complying with GMC standards, NHS policies, safeguarding requirements, and practice governance procedures.

Job responsibilities

Clinical Duties Conduct face-to-face, telephone, and home visit consultations in line with practice requirements.

Assess, diagnose, and manage patients presenting with a wide range of health conditions.

Make autonomous clinical decisions and develop appropriate treatment and care plans.

Screen for disease risk factors and promote health education and preventative care.

Prescribe safely and in accordance with local and national prescribing guidelines.

Maintain accurate, timely, and contemporaneous clinical records.

Manage referrals, admissions, discharges, investigations, and follow-up care as appropriate.
