Position: Personal Wellbeing Assistant (multiple)

Main duties of the job
  • This may include personal care, assist with medication, support with meals and drinks.
  • You will be required to have a genuine interest and commitment to providing high quality care and support to adults in promoting their privacy, dignity, choice and independence.
  • These are part time positions only, working 21 hours per week and therefore the salary will be pro rata as shown above.
  • You will be required to work on a rota basis which will include weekends.
  • This post includes working evening shifts 4 pm- 10pm; we predominantly require evening shifts.
  • The role will require you to be able to travel across the city.
  • A rolling programme of induction and accredited training opportunities will build your confidence and enable you to reach your full potential.

Key Duties / Responsibilities

1.

Working in a person‑centred way to promote and maximise independence, understanding the individuals needs, their aspirations and goals.

2.

Contribute to and inform individual care act assessments and individualised care plans to enable individuals and reduce any long‑term care needs.

3.

Encourage or assist adults where appropriate with personal care, such as bathing, showering, toileting, hygiene continence care, oral hygiene, skin integrity, meal prep (nutrition and diet) and administering medication including controlled drugs.

4.

To apply infection control and food hygiene measures within the adults home environment and to use protective equipment.

5.

Enabling and support maybe include activities as supervising, monitoring and coaching to empower adults to be as independent as possible and have control over their own lives. You may also be required to escort or accompany adults to outings, appointments or other events. This also includes tailored exercise programs.

6.

Undertake commercial activities including cleaning tasks to maximise the efficiency of the service

7.

To work in partnership with other relevant professionals and external agencies to provide a personalised service to meet individuals needs.

8.

Maintain high standards of care and support, in a safe environment which promotes equality culture, disability, race, age, sexuality, marital or civil partnership status, transgender status or religious belief and sensitivity for all adults.

9.

Flexibility in ensuring that the city council can be a provider of last resort in emergency situations

10.

Use the councils electronic monitoring record system to maintain a record of work undertaken.

11.

Work with autonomy to ensure Enablement and Support Plans accurately reflect the persons needs and agreed level of support by informing management of any changes, always seeking to optimise the participation of the person accessing the service in the design and planning of the care and support they require, take prompt action to…
