Pharmacy Technician – Discharge to Assess Pathways/Community Services

Midlands Partnership University NHS Foundation Trust is a unique integrated health and social care provider across Staffordshire, Stoke‑on‑Trent and Shropshire. This role leads the development and delivery of an Integrated Home First Medicines Optimisation Service, improving safe transfer and use of medicines across clinical interfaces, meeting legal and Good Practice Standards, and supporting safe administration of medicines in community settings.

• Work as part of a multidisciplinary team within the North and South Home First Hubs and support the Lead Pharmacist in establishing and managing care pathways for patients referred to the Home First Service with medication‑related problems for assessment, support, and follow‑up.
• Engage in cross‑sector work in the community with multiple agencies and Primary Care Networks (PCNs) to support complex medication reviews for patients in their own homes.
• Lead service development and delivery to agreed targets, collaborating with stakeholders across the Trust.
• Coordinate and ensure compliance with CQC and other agency Good Practice Standards, and NICE NG 67 recommendations.
• Provide administrative support, data collection, and reporting for service performance evaluation.

• BTEC and NVQ3 (or equivalent) in Pharmaceutical Science.
• Registered as a Pharmacy Technician with the General Pharmaceutical Council.
• Nationally recognised qualification as an accredited Medicines Management Technician.
• Nationally recognised Accredited Checking Technician Certificate.

• Knowledge of principles of medicines optimisation.
• Experience in a broad range of hospital/community pharmacy activities, including dispensary management and clinical pharmacy activities.
• Staff management experience.
• Excellent interpersonal and IT skills; GPHC registered pharmacy technician.

37.5 hours per week, Monday to Friday, with limited late shift duties and Bank Holiday commitments. Flexibility may be required.
