Location: Stoke-on-Trent
Kingston and Richmond NHS Foundation Trust is seeking a Band 6 Community Diabetes Specialist Nurse to deliver high-quality diabetes care in community settings across Kingston. You will work within the Tier 3 Diabetes team, providing person-centred care and supporting self-management.
You will undertake clinical assessments, develop care plans, mentor colleagues, and liaise with GPs, dietitians and primary care staff to reduce inequalities for people living with diabetes.
