Northern Case Management is seeking an Associate Clinical Case Manager to join our team in the Bury & North West area. The role is full-time, 37.5 hours per week, with hybrid working, and involves managing case loads for paediatric and/or adult clients in home and community settings.
You will assess needs, plan rehabilitation, recruit and supervise care workers, and ensure high-quality, client-centered services while complying with CQC standards. Travel across multi-site locations is expected.
