- Conduct thorough strengths-based assessments to develop personalised care plans
- Recommend necessary equipment and adaptations to promote independence
- Carry out home visits across Brent, working closely with residents to implement effective reablement strategies
- Collaborate with Social Workers, Care Assessors, Enablers, and health professionals to ensure coordinated, person-centred care
- Maintain strong partnerships with hospitals and community organisations to facilitate seamless service delivery
- Be present in the borough for at least three days each week, working in diverse settings like customers’ homes, community locations, partner sites, and the Civic Centre
- Degree or equivalent in Occupational Therapy
- Recent UK experience as an Occupational Therapist
- Experience with in the NHS
- Registered with the HCPC
