Overview

We are looking to recruit an enthusiastic and highly motivated Advanced Nurse Practitioner to join our friendly and progressive practice team.

We are a four-site practice providing high-quality primary care services to a patient population of approximately 17,000 across the Longton and Meir areas of Stoke-on-Trent.

Responsibilities
  • Work as an autonomous practitioner within the multidisciplinary team, delivering high standards of clinical care.
  • Undertake the assessment, diagnosis, and management of patients presenting with a wide range of minor illnesses and injuries, as well as acute and long-term conditions.
  • Practice safely and effectively within your scope of practice, demonstrating clinical decision-making and leadership skills.
  • Initiate appropriate investigations, treatment plans, and referrals in line with local and national guidelines.
  • Provide safe, effective, and evidence-based care, managing a broad spectrum of undifferentiated and complex presentations.
  • Exercise independent prescribing, initiating, reviewing, and monitoring appropriate treatment plans.
  • Recognise the limits of practice, seeking advice or referring patients to other healthcare professionals and specialist services when appropriate.
  • Ensure patients are fully informed about proposed treatments and procedures, obtaining valid consent in line with professional and legal standards.
  • Maintain accurate, timely, and comprehensive documentation of all consultations and clinical decisions within the EMIS Web clinical system, in accordance with local and national policies.

We are a friendly and progressive practice with a combined patient population of approximately 17,000, operating across four sites.

You will be joining a well-established, multidisciplinary clinical team and will play an integral role in delivering high-quality, patient-centred care.

We are an accredited training and teaching practice, working in partnership with the West Midlands Deanery and Keele University, supporting the development of future healthcare professionals.
