Practice Nurse (Long Term Condition Management)

Location: The Potteries Surgery, Reading

Salary: Up to £48,000 depending on experience

Hours: 25 – 37.5 per week

• Assist the registered manager with CQC outcomes and ensure practice compliance with regulations.
• Collaborate with the general practice team to meet patient needs and uphold practice policies and procedures.
• Provide care for presenting patients, including chronic disease management, treatment and evaluation of care.
• Support the development and delivery of long‑term condition management services.
• Assist with cervical cytology and baby immunisation duties, where required.

• UK registered nurse with significant post‑registration experience.
• Experience in cervical cytology and baby immunisations.
• Good knowledge of the Quality Outcomes Framework.

• Annual salary up to £48,000 (subject to experience).
• 27 days annual leave plus bank holidays, pro rata.
