About the Role:

Liquid Personnel is seeking an Adult Social Worker to join our client’s Community Wellbeing team based in Stoke-on-Trent. As an Adult Social Worker within the Community Wellbeing Team, you will support adults aged 18+ with care and support needs arising from physical disabilities, mental health conditions, learning disabilities, or age-related challenges. The role focuses on early intervention, person-centred planning, and strength-based approaches to promote independence and wellbeing in the community.

What will your responsibilities be?

In this role, you will

  • Carry outCare Act 2014 assessments to identify eligible needs and support planning.
  • Applystrengths-basedandperson-centredapproaches to identify individual goals and outcomes.
  • Coordinate and review support packages, ensuring they are proportionate, cost-effective, and meet statutory requirements.
  • Completesafeguarding enquiriesunder Section 42 where concerns arise.
  • Undertake Mental Capacity Act assessments and support Best Interest decision-making where appropriate.
  • Work closely withhealth professionals, care providers, families, and community organisations to develop holistic support plans.
  • Promotereablement, prevention, and independence, making use of local resources and technology-enabled care.
  • Maintain accurate and timelycase recordsusing council systems such as Liquid logic.

Qualifications & Experience:

To be successful in this role you must have,

  • Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW.
  • Registration:
    Must be registered with Social Work England.
  • Experience:
    Substantial experience working with vulnerable adults, including those with complex needs.
  • Legislation Knowledge:
    Strong understanding of theCare Act 2014,Mental Capacity Act 2005, and safeguarding procedures.
  • Skills:
    Excellent assessment, analysis, and communication skills. Ability to work collaboratively in a multi-disciplinary setting.
  • Driving Licence:
    Full UK driving licence and access to a car is often required for community visits.
