Position: Senior Social Worker (Fairer Funding Team)
Location: Stoke-on-Trent

To offer management support and supervision to the Adult Social Work services to ensure that a strengths‑based approach in line with the principles of the Care Act 2014 is applied within all areas of Social Work practice.

Main duties of the job

Key Responsibilities:

  • Undertake holistic social care assessments in line with the Care Act 2014 and CHC National Framework.
  • Participate in NHS Continuing Healthcare assessments, including completion of Decision Support Tools (DSTs).
  • Contribute professional social work expertise to multidisciplinary team discussions.
  • Safeguard adults at risk and ensure appropriate safeguarding procedures are followed.
  • Support discharge planning and prevent delayed transfers of care.
  • Work collaboratively with NHS colleagues, care providers, housing, and voluntary agencies.
  • Maintain accurate, timely, and legally compliant case records.
  • Promote independence, choice, dignity, and person‑centred practice.
  • Assess mental capacity and apply the Mental Capacity Act 2005 appropriately.
  • Manage a complex caseload and prioritise workloads effectively.
  • Provide advice and support to individuals, carers, and families regarding CHC processes and eligibility.
  • Attend panel meetings, case conferences, and reviews as required.
  • Ensure services are delivered within statutory responsibilities and local policies.

Person Specification Qualifications
  • Recognised Social Work Qualification
  • Registered or be in the process of registering with Social Work England
  • Practice in accordance with Social Work England Standards
