, where your work has an immediate and meaningful impact.
At Leaders in Care, we understand the commitment and challenges that come with a career in social work. That's why we work with the onboarding teams to help give you a smooth transition into a stable permanent role.
This role also comes with great benefits - including hybrid working. Enjoy up to two days a week from home, giving you the flexibility to balance work with your personal and family life.

Your role will involve completing assessments within 14-28 days
, undertaking 6-12 week reviews
, and contributing to a light-touch triage system that ensures people receive the right level of support at the right time. You'll play a key role in ensuring smooth transitions from hospital to home, promoting independence and reducing readmissions.
