We are seeking a compassionate and proactive individual to join our team in Nottinghamshire as a Street Outreach Recovery Co-ordinator
, dedicated to supporting people who are rough sleeping or at risk of homelessness. You will work across nights and weekends, delivering street outreach and night shelter support to help individuals make positive changes and move towards stable accommodation. This is a hands‑on role that involves engaging with people at all times of day and night, building trusting relationships, and empowering them to express their strengths and aspirations while taking steps towards a more settled future.

Where: Nottinghamshire (the role requires cover the whole county)

Hours: 37.5 hours per week

Contract Type: Fixed Term until 31st March 2028

Full‑time hours at Change Grow Live are 37.5 hours per week. For part‑time roles, the salary and payments will be pro rata based on contracted hours

Responsibilities

With this role you’ll be working as part of our Nottinghamshire county street outreach team.

The team supports people in the community who have experience of rough sleeping/homelessness and are using drugs and/or alcohol.

No two days are the same and we treat every person who comes into our service as an individual, so being flexible, curious and a good listener are key.

This role includes all weather work, requires flexibility to cover early morning and evening outreach shifts, driving our health bus and being assertive in engaging people who services may have not been able to in the past.

You’ll have to record information about what you do, so enjoying a bit of paperwork and attention to detail is important. But most of the time you’ll be enjoying working with people from all backgrounds, making a difference to their lives and working in partnership with other providers.

About You:

  • Experience of engaging and providing meaningful support to individuals with severe multiple disadvantages.
  • Experience of communicating effectively with people requiring support
  • Experience of recording relevant sensitive information in a timely manner
  • Knowledge of at drugs and alcohol support and at least one of the following; housing; homelessness; the benefits system, mental or physical health, the criminal justice system, domestic abuse
  • Experience of supporting people who may be affected by homelessness, mental health and substance use.
  • Knowledge of Safeguarding Procedures and their application.
  • Experience of working with people in crisis
  • Knowledge and experience of delivering trauma informed support
  • To be able
    • To work as part of a multi-disciplinary team
    • To have excellent Interpersonal Skills / Ability to build rapport
    • To work independently; adhering to local risk management policies
    • To problem solve in challenging situations
    • To be service user focused, and service user led in support planning.
    • To walk for several hours each day to complete the outreach activity
    • To manage individuals that have multiple unmet needs and take support to the individual
    • Full driving licence and willingness to drive the Outreach Van
