We are seeking a compassionate and resourceful Health Navigator to join our team and support individuals in accessing essential community programs and services. This role is vital in building trust and providing practical guidance to help people navigate daily needs such as food access, transportation, housing concerns, and health information. Key Responsibilities:- Establish rapport with individuals seeking assistance and serve as a reliable point of contact
- Provide general guidance on available programs and local supports tailored to individual needs
- Assist clients in understanding health-related information, such as blood pressure readings
- Help individuals locate resources such as groceries, transportation services, and housing support
- Facilitate connections to appropriate programs based on client situations and needs
- Support clients in navigating processes to access services and resources efficiently
- Maintain accurate records of interactions and resource referrals
- Collaborate with community organizations and service providers to enhance resource availability
Qualifications:
- Excellent interpersonal and communication skills with a compassionate approach
- Strong understanding of community resources related to health, housing, transportation, and food access
- Ability to assess individual needs and provide appropriate guidance
- Experience in social services, healthcare, or community outreach preferred
- Ability to work independently and as part of a team
- Organizational skills and attention to detail
- Bilingual skills are a plus but not required
- High school diploma or equivalent; additional training in social services or health navigation is advantageous
