- Schedule:
Monday - Friday
- Provide physical therapy services to patients in accordance with a written physician's plan of care and the state Physical Therapy Practice Act. This includes assessing and evaluating therapeutic/rehabilitative/functional status, evaluating the home environment, and determining the potential for rehabilitation, including muscle strength, mobility, gait, and range of motion.
- Initiate and develop individualized physical therapy programs for patients while also instructing other personnel and/or family/caregiver members involved in the patient's care. Actively participate in case conferences to ensure a comprehensive and coordinated treatment approach.
- Instruct patients and their families/caregivers in the use and care of therapeutic appliances and, when necessary, train patients in the use of prosthetic devices. Additionally, provide in-service education programs for nursing organization personnel as needed.
- Participate in discharge planning for patients, identifying their ongoing needs and referring them to other home health services as required.
- Prepare and submit clinical progress summaries based on the achievement of goals, following the guidelines set forth by the Organization policy.
- Offer physical therapy assistant consultation to home families/caregivers when indicated, and actively engage in the peer consultation process to enhance the overall quality of care.
- Possess a degree as a physical therapist approved by an accredited organization.
- A minimum of two years of relevant experience as a physical therapist assistant, with preference given to those with community/home health experience is desirable.
- Demonstrate excellent verbal and written communication skills, as well as organizational abilities.
- Maintain current BLS CPR certification.
- Possess a valid driver's license and own a reliable, insured vehicle.
