Job DescriptionTempus Unlimited, Inc. is a nonprofit organization that provides community‑based services to empower children and adults with disabilities to live as independently as possible in the least restrictive environment. The agency encourages the inclusion of people with disabilities into the mainstream of society through social, recreational, family, and work activities. The Adult Foster Care (AFC) Supervisor provides support, training and coaching to AFC caregivers and program members, ensuring quality care through support to Care Managers, site visits, and oversight of the Community Services Adult Foster Care program.
The AFC program offers a home‑based care setting for individuals needing assistance with daily activities.
- Provide direct support to the care managers and nurses in the community.
- Conduct site visits to consumer homes and produce quality feedback/reports per the manager’s guidance.
- Offer input to annual performance evaluations.
- Provide monitoring and oversight for efficiencies with all AFC referrals.
- Lead and assist with program team meetings.
- Support and train staff during orientation or initial planning meetings to ensure proper efficiencies in services.
- Support Community Living Program Manager as assigned.
- Monitor LTSS process to ensure accuracy and timely submissions.
- Assign caseloads.
- Provide supervision to nurses and care managers.
- Handle member or caregiver complaints.
- Support new program development and implementation.
- Perform additional responsibilities as assigned.
- Three or more years of recent experience working with the elderly or disabled.
- Ability to supervise and direct staff.
- Excellent organizational skills.
- Attention to accuracy and detail.
- Strong interpersonal skills, including telephone skills.
- Excellent oral and written communication.
- Good computer and analytical skills.
- Innovative thinking.
- Ability to maintain confidentiality.
- Perform all other duties as required.
- Supervisory experience.
- Bachelor’s degree required.
- A Human Services degree preferred but can be substituted with a related degree plus field experience.
- This job operates in a professional office environment and routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines.
- The physical demands described are representative of those required to perform the essential functions of this job.
- The employee is regularly required to talk and/or hear, sit; stand; walk; use hands to handle or feel; and reach with hands and arms.
- Travel is required; must have a valid driver’s license and reliable transportation.
