Eventus Whole Health delivers an integrated model of care to adults who reside in skilled nursing and assisted living facilities through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants and in-house Support Staff.
Responsibilities- Provide primary‑care services in accordance with Eventus Whole Health site‑specific protocols and consistent with the standard of care for family medicine, geriatric medicine, and/or internal medicine as it pertains to long‑term care facility work.
- Review and sign MARs as requested by facilities; sign all orders specific for primary care while at the facility and/or within 24 hours if faxed or emailed to include phone orders, DME orders, Death Certificates, Plans of Care, PT/OT orders, hospice orders as appropriate, and other orders as relevant and appropriate to the facility patient care needs and Eventus Whole Health.
- Review labs within 24 hours of being sent and manage all narcotic prescriptions in accordance with Eventus Whole Health Narcotic Rx Policy.
- Provide on‑call triage duties per the on‑call triage protocol policy; adhere to the latest agreed‑upon triage schedule.
- Complete documentation and submit billing records to Eventus Whole Health according to published charting and bill sheet submission policies; provide facilities with a hard copy of the electronic documentation within 48 hours of service.
- Establish and maintain open, positive communication with facility staff and administration; provide verbal or written instructions or feedback regarding medications and other pertinent caregiver information.
- Adhere to Eventus Whole Health standards, policies and procedures, legal and regulatory agencies' rules and guidelines, and professional ethical standards.
- Maintain patient confidentiality at all times; comply with HIPAA rules and regulations.
- Consult with the clinical supervisor before refusing a resident, family, or facility request or recommendation regarding the care of a resident.
- Master’s degree in Nursing from an accredited institution and current, valid license to practice medicine issued by the state Medical Board.
- Or:
Graduated from a physician assistant educational program accredited by a National Commission on Accreditation of Allied Education Programs, passed the national certification exam, licensed as a Physician Assistant in the state of residence, and specialization in geriatric, family, or psychiatry desired.
