Visit patients one-on-one in their home, provide teaching and education, and use all your nursing skills. Qualified applicants will have:

Active State of CT LPN license in good standing

Minimum of one year of experience, homecare experience preferred

Ability to be organized and dedicated to quality patient care

Aptitude for learning electronic documentation system

Flexibility and adaptability in meeting job requirements

Minimum one year of clinical experience, experience in home care preferred

Ability to communicate well and build rapport with patients and families

Ability to take direction well

Full time positions, some weekends required
