Hours – 30 hours per week(Including weekends, on a flexible rota)
About the Role

We are seeking an experienced Registered Nurse to coordinate our Hospice at Home service, ensuring patients receive high-quality end of life care in their own homes. The role combines clinical leadership, caseload management, staff supervision and partnership working to support patients, families and carers.

Key Responsibilities
  • Manage referrals, assessments and ongoing caseloads.
  • Coordinate care and work closely with multidisciplinary teams and external partners.
  • Support patients and families, enabling people to remain at home where possible.
  • Lead, mentor and line manage Hospice at Home nursing and care staff.
  • Oversee staff recruitment, induction, supervision and performance reviews.
  • Support service planning, rostering and allocation of care based on clinical need.
  • Maintain accurate patient records and contribute to service governance, audits and quality improvement activities.
  • Deputise for the Hospice at Home Manager when required.
About You

You will be a Registered Nurse with:

  • A current NMC registration (PIN).
  • Significant post-registration nursing experience.
  • Experience working in community and/or palliative care settings.
  • Previous experience supervising or managing staff.
  • Excellent communication, organisational and leadership skills.
  • Confidence in managing clinical priorities and working collaboratively with healthcare professionals.
  • Good IT skills, including Microsoft Office and electronic patient record systems.

You must have access to a car and have a full clean driving licence.
