This is a two year fixed contract funded by Gloucestershire County Council as part of a pilot project. We are looking for someone who can work 22.5 hours per week over 4 days at £29,000 pro rata per annum.
Who We AreThe Grace Network is a series of place-based hubs, blending social enterprise with community transformation. The hubs success fuels deeper social impact, reinvesting surplus revenue into local initiatives, increasing staff pay, and providing essential goods and services at accessible rates.
By developing sustainable operational systems, The Grace Network strengthens communities and creates thriving, resilient enterprises. We are a network of hubs and enterprises across Gloucestershire and Bristol, working to rewire the local economy so that it serves people, not the other way round. We exist to meet people in both the everyday moments of life and the moments of crisis.
From shared meals to emergency provision, from belonging to survival, we aspire to build systems that hold people through the full range of human experience. Job Description This role sits at the centre of our Crisis Resilience Fund (CRF) work across Stroud and Cotswold Districts.
You will act as the bridge between community hubs, referral partners, and individuals in crisis. Your job is to make sure no one falls through the gaps and no one has to tell their story twice. You will develop and strengthen relationships with appropriate support services and external partners to establish effective referral pathways, ensuring that people in need can access timely physical, practical, and holistic support.
This role will develop and implement a robust recording and monitoring system to capture referrals, outcomes, and the social impact of the service. This will provide the evidence base for evaluating the effectiveness of the service and support accurate reporting of compliance with the CRF contract requirements for Gloucestershire County Council. Utilising The Grace Network's economic power to address safety and physiological needs, you will facilitate access to essential household furniture, household goods, children's toys, meals, and other practical resources.
The Role Partnerships & Referral Pathways- Build and maintain referral partnerships with community organisations, Gloucestershire County Council and County Council commissioned groups, local authorities, the NHS and voluntary sector.

- Use local intelligence and community feedback to target support where it is most needed, including outside hub locations.
- Track referrals, outcomes and social impact, maintaining accurate records and reporting.
- Use data and insights to monitor service effectiveness and inform continuous improvement.
- Support the development of a scalable Community Response Framework (CRF) model across the county.
This is a new and evolving role, created to support the delivery of a new contract.
As such, the postholder will be proactive, adaptable, and comfortable working in an environment where services and processes are still being developed. They will be able to respond dynamically to emerging needs, using initiative and sound judgement to shape and refine the service as it grows.
The successful candidate will be flexible in their approach, recognising that priorities may change as we gain a better understanding of the needs of the communities we serve.
They will be confident in developing new ways of working, establishing effective referral pathways with partner organisations, and building strong collaborative relationships to improve access to support. They will also be committed to continuous learning and service improvement, developing systems to record activity, measure outcomes, and capture the social impact of the service. This evidence will…
