Sage Group Health Care  Staffing is seeking a Home Health Registered Nurse for a 13-week travel/contract position in Sturgeon Bay, WI. We are accepting both local and traveler candidates.

Pay Rate:

Range $2000 to $2080 Weekly Gross Pay based on a 40-hour work week (Includes Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)

Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments

• Registered Nurse: 2 years - Required
• Recent/Current Home Health Nursing experience: 1 year within the past 3 years - Required
• OASIS experience - Required
• Home Care Home Base (HCHB) charting experience - Required
• Must have valid Driver’s License and Personal Auto Insurance

• ASN or an equivalent – Required
• BSN or an equivalent – Highly preferred
• RN License for State applying or Compact if accepted in the state
  - Required
• BLS Certification from AHA or American Red Cross
  - Required

• Provide and coordinate care for assigned patients in comfort of their homes.
• Develop and implement nursing care plans including Start of Care and admissions as assigned.
• Support patient recovery andability to stay at home - enabling them to spend quality time with their loved ones and doing the activities they enjoy
• Empower patients and families toreach healthcare goals by educating them on disease management
• Case management and provide clinical oversight to interdisciplinary
• Provide nursing care and services within the standards and scope of nursing practice as defined within thestate Nurse Practice Act regulations, policies, procedures, and protocols,as well as facility and corporate policies and procedures.
• Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
• Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
• Notify physicians of significant changes in status
• Document and complete charting for all patient visits within 24 hours.
