In this vital position, you will provide holistic health services to diverse age groups, emphasizing collaborative care within a multidisciplinary environment. Your responsibilities will include assessing, diagnosing, and treating clients, while also managing chronic disease interventions. As part of the role, you will focus on integrating cultural sensitivity into your practice and providing community education.

Key Responsibilities:

• Offer primary care including assessments and treatment

• Collaborate with clients on chronic disease management

• Conduct procedures and diagnostic testing as required

• Coordinate mental health support and referrals

• Engage in community health initiatives and workshops

Requirements:

• Master's degree in Nursing or recognized NP program

• Valid registration with College of Nurses of Ontario

• Minimum 2 years of NP experience preferred

• Strong clinical and interpersonal skills

• Familiarity with Indigenous health initiatives beneficial
